Guidance Forms

The forms listed below are currently used by the Guidance Department and can be downloaded and printed using Adobe Acrobat Reader.


Third Party Release Form

This form is used to allow NHS to send information regarding a student upon request.

Secondary School Report

The Guidance Department utilizes this teacher reference form; it is not released to schools.

Senior Self-Evaluation

This form is completed by seniors and utilized by the Guidance Counselor to enhance a Counselor recommendation.

Transcript Request Form

Students should list all colleges that they are requesting a transcript be sent to. No transcripts will be released without being listed on this form.

Sample Resume

This is a sample resume that students can use as a guide in creating their own personal resume.

Community Service Log

This form should be utilized by students to record their community service hours. Once complete, they may be turned into Guidance.

Community Service Letter for Portfolio

 This form should be printed and brought to Guidance for verification. Once verified, this form can be scanned into a student's electronic portfolio.

College List Information Log

This document can be used to organize college admission information and classify colleges based on this information.

Parent Brag Sheet For
College Recommendation

We are requesting this form be completed by parents and returned to their child's guidance counselor. This will assist the counselor in writing the letter of recommendation.
